Write a survey of at least 8 close-ended questions which will be asked of participants in your study.  This will be the quantitative methods portion of your proposal.  This survey should include at least one question or set of questions developed by another researcher. Before beginning to write or select your survey questions, create a list of at least two concepts that you intend to measure with the questions on the survey. Describe those concepts in the same area where you submit your link to the form. The survey should be intended for administration to the general public as a face-to-face interview. The questions and layout of the survey should reflect the fact that you will be reading them out loud to the respondents. After thinking about the questions, create your survey using a Google form.  An overview of the process is here. Remember that a close-ended question is one where you limit the number of possible responses and the respondent has to pick from one of them (an exception to this rule is if your question is “Check all that apply”).  Do not limit yourself to simple yes/no questions, which only captures whether or not a concept or variable is present.  Close ended questions can also be used to capture frequency (How many times in a week), variety (favorite ice cream flavor), and intensity (strongly agree/agree/disagree/strongly disagree) among many possible options. Read your survey out loud before handing it in and change any items that don’t “speak” smoothly or comfortably. You should use at least one survey question created by another researcher. The top of the survey should say (fill in the blanks with appropriate information): (Instructions to interviewers are enclosed in parentheses. When doing the interviews, read out loud all of the text that is not in parentheses exactly as it is printed.)
