Community Needs Assessment
During this clinical rotation, you have engaged in service-leaning activities in collaboration with our community partners. For this scholarly paper, you will complete a community needs assessment, based on one of these activities. Utilizing the community assessment components (People, Place/Environment, and Social systems), address the guidelines listed below: Describe the Community 1. People (Demographic, Biological Factors, Social Factors, and Cultural Factors) 2. Place/Environment (Physical Factors, Environmental Factor) 3. Social Systems (Health Systems, Economic Factors, Resources and Services) Research and Analysis of the Community Data Collection (Conduct a Windshield survey) Analysis of Community Assessment Data (Identifying Community needs and strengths) Community Health Program (Identify a resource that this community could utilize to help alleviate the needs identified) Summarize how your service-learning activity helps meet the Healthy People 2020 initiatives. Reflect on what was learned from the assignment and how it will impact your practice in the future. Requirements: Your paper must be APA format and at least 750 words not to exceed 1000 words. Double space your paper and use Times New Roman 12-point font and 1” margins. Proofread your paper; visit and present proof of seeing a writing tutor before submitting your paper to “Turn-it-in”. A minimum of two (2) scholarly references (ie: Healthy People 2020) must be used. APA format must be used in citing and referencing sources. QEP WRITNG: 1. Students are expected to complete a written paper during their clinical rotation using one of their assigned clients. 2. Do not violate HIPPA in writing this paper. 3. Include the identified needs, the importance of this topic, and the relevance to nursing.
