Case Study Three – Mr. NielsenDent 202- Oral Hygiene and Risk Factor AssignmentMr. Lyle Nielson, a 67-year-old male, presents to the SAIT Dental clinic for a preventiveprocedures appointment. He has been a patient of both the SAIT Dental Clinic and Dr. Tooth forthe past 15 years. However, it has been 18 months since his last visit to eitherclinic. His wifedied about 6 months ago; he is slowly coping and getting back to his normal routine.Dental HistoryMr. Nielsen tries to brush and floss daily, but finds it difficult and has trouble using floss due tolimited dexterity in his hands. He is able to grasp a regular toothbrush and does not have thefinancial means, at this time, to purchase an electric toothbrush. He is using a very largetoothbrush with soft bristles which does not fit properly in the posterior regions of his mouth. Heuses his toothbrush in a circular motion, often missing the cervical regions.You note abundant plaque biofilm and materia alba along the cervical third of all teeth. He hasgeneralized recession and large embrasure spaces throughout his mouth. Mr. Nielsen finds hisgums often bleedwhen he brushes and flosses. Mr. Nielsen uses fluoridated toothpaste.Mr. Nielsen’s pocket depths range from 2-5 mm in depth. There is generalized bleeding uponprobing in the posterior areas of his mouth.Mr. Nielsen has numerous old occlusal and interproximal restorations in his mouth and thedentist notes 3 new areas of recurrent decay. He has porcelain fused to metal crowns on teeth1.1, 2.1, 2.2, 2.6 and 4.6. His wisdom teeth were removed over forty years ago, as there wasnot enough space for them to erupt. All other teeth are present and no tooth mobility is present.Medical HistoryMr. Nielsen has mild arthritis in his hands which is causing him to lose some of his dexteritywhen using a toothbrush or floss. Occasionally, he takes an anti-inflammatory such as Advil, forpain associated with his arthritis.
